กลไกการทำงาน / การออกฤทธิ์ของ Cerebrolysin
ยาซีรีโบรไลซิน (Cerebrolysin) เป็นสารสกัดบริสุทธิ์จากสมองของสุกร
มีส่วนประกอบของสารประเภทเปปไทด์ (Peptide), นูโทรปิกแฟคเตอร์ (Nootropic factor,
สารที่ช่วยในการบำรุงเซลล์สมอง), สารปรับการเจริญเติบโตของสมอง (Nerve growth factor) , และซีเลียรี
นิวโรโทรปิก แฟคเตอร์ (Ciliary neurotrophic factor, สารช่วยสร้างสารสื่อประสาท),
ด้วยองค์ประกอบที่กล่าวมา ทางการแพทย์จึงได้นำยาซีรีโบรไลซินมาบำบัดอาการทางสมอง เช่น
ผู้ป่วยที่มีภาวะเส้นเลือดในสมองตีบ หรือแตก (Stroke), รวมถึงผู้มีภาวะความจำเสื่อม
/สมองเสื่อมด้วยพยาธิสภาพหลอดเลือดในสมอง (Vascular dementia) อย่างไรก็ตาม
ประโยชน์ทางคลินิกดังกล่าวยังต้องการข้อมูลทางการแพทย์ มาสนับสนุนอีกมากพอสมควร
กลไกในการเกิด reperfusion injury ของสมอง ได้แก่ การเกิดภาวะ oxidative stress, inflammation,
BBB injury หรือเกิด necrosis โดยกระบวนการเกิด ischemic cell death ในสมองเกิดได้จากหลายปัจจัย เช่น

และ cell death ในสมองได้หลังจาก 6 ชั่วโมง ถึง 72 ชั่วโมง ซึ่งอยู่ในระยะ acute stage ในช่วงนี้จะเกิดภาวะ
neuroinflammation และ reperfusion injury ต่อมาในช่วง 72 ชั่วโมง ถึงระยะเวลา 6 สัปดาห์ ในระยะ
subacute stage จะพบภาวะ neuroprotective inflammation, neoangiogenic และ recovery process
ซึ่งหลังจาก 6 สัปดาห์ ในระยะ chronic stage สมองจะเกิดกระบวนการต่างๆ ที่เกิดขึ้นภายในสมอง
มีทั้งกระบวนการ endogenous defense activity (EDA) ซึ่งเป็นกลไลการปกป้องของเซลล์ประสาทในสมอง
และนำไปสู่ผลลัพท์ของการรักษาที่ดี ได้แก่ กระบวนการ neuroprotection, neurotrophycity,
neuroplasticity, neurogenesis แต่อย่างไรก็ตาม พบว่า damage mechanism ต่างๆ
ที่เกิดขึ้นในสมองอาจนำไปสู่ผลลัพท์ในการรักษาที่แย่ลง ดังนั้น เป้าหมายในการรักษา ischemic stroke ด้วยยา
